Better Birth Outcomes- Comprehensive is a statewide program that supports pregnant and parenting families in connecting with healthcare and community resources.
Through one-on-one nursing support and care coordination, families receive:
- Comprehensive nursing assessments
- Assistance connecting with healthcare
- Identify and address barriers to care
- Perform prenatal/postnatal depression screenings and developmental screenings
- Ongoing follow-up throughout pregnancy and up to six months after birth
- Referrals to community resources for family needs, including food, clothing, childcare, transportation, etc.
