2026 Permit Application

Please feel free to contact the Monroe County Health Inspector with questions & concerns.

 

2026 Food Establishment Compliance Application

All applications and fees shall be submitted to the Health Department postmarked by December 31, 2025. Late fees will be imposed on applications and/or renewal fees postmarked after December 31, 2025. Late fees are $100 for the first month, and $50 each additional month. Late fees are cumulative.

Examples for applications/renewal fees postmarked in:

January 2025 = renewal fee + $100

February 2025 = renewal fee + $150

March 1 – March 9, 2025 = renewal fee + $200

After March 10, 2026, you will be referred to the States Attorney’s office.

Annual Fees:

High Risk $309

Medium Risk $206

Low Risk $103
