2024 Food Establishment Compliance Application

Annual fee along with renewal application is due no later than 12/31/2023. 

Late fee increases by $25 per month (i.e., $210 fee received 1/1/2024 – 1/31/2024 must add a $25 late fee bringing your balance to $235, so on and so forth. After 3/31/2024, you will be referred to the States Attorney’s office.)

Medium Risk $116

Low Risk $56
